OASIS Physical Therapy

It is with a heavy heart that we inform you of the upcoming closure of OASIS Physical Therapy. This decision was not made lightly, and we understand the impact it may have on your ongoing care and rehabilitation journey.

Despite this closure, we want to assure you that our commitment to your health and well-being remains our priority. While our PT clinic will no longer be operational after May 31, 2024, we are dedicated to ensuring a seamless transition for all our patients to alternative, reputable physical therapy providers. We are here to support you every step of the way. Our team will gladly assist you in locating a clinic that meets your specific needs and preferences, and which merit our full confidence. Should you have any questions or concerns during this transition period, please do not hesitate to contact us.

Your medical records will remain safe in our electronic medical records (EMR) system. We will securely send copies of needed records with your physical therapy referral. If you need to obtain your medical records, please contact our office at 972-335-8455.
